Our Model
Established in 1978 in response to the health care crisis in the Puerto
Rican community, the Hispanic Health Council (HHC) is a community-based,
non-profit organization located in Hartford, Connecticut. Sustained over
28 years by the combined efforts of community activists, researchers
and dedicated staff, HHC has earned the trust and confidence "confianza"
of the community and is a valued institution in the region.
We are proud of our history and collective accomplishments. HHC's unique
organizational model integrates public advocacy, high-level community-based
research and research-based service to provide outstanding care to the
poor and underserved. To carry out this charge, the organization has
established six Centers of Excellence. Each Center provides quality community
services in their respective area of specialization. The work is evaluated
using state-of-the-art methodologies. Program developments, outcome measurements
and new learnings are shared with other social service providers. The
Centers integrate research, direct service, training and advocacy.
How it Works
Community based research listens to the people and reflects their needs.
It tells us:
• The size and the shape of our community
• What their problems are and what they need to be well, and to succeed
• How they see the future for themselves and for their children
• How to add new knowledge and understanding to the study of urgent public
health issues
Our research based services respond to the community. They offer
• Education that’s focused on real needs
• Prevention that gets to the heart of the issue.
• Coordinated networks of support.
• Integrated within a bilingual, multi-cultural environment
• New models of service that are effective in bringing about change
Our advocacy clarifies the issues and mobilizes the community for change.
• We have an authentic voice based in 28 years of practice.
• We are trusted in the community because of our service
• We have a record of effectiveness
because of our experience and multi-disciplinary
expertise.
• Our history inspires.
